feat: Initial PMM implementation

Introduce the foundational Physical Memory Manager (PMM) and restructure the project for modularity.
Implemented basic PMM and related memory management headers (mm/pmm.h, mm/vm.h).
Updated Makefile and build process to support new modular structure.
